Intent to Re-enroll at a School of Choice
for the 2017-2018 School Year

Required Information
The time has come for our Schools of Choice to begin planning for the space and staff requirements for the 2017-2018 school year.
To facilitate the planning process we need a firm commitment from you and for your student.
Due to a large number of applications we receive each year, we want to be certain that we retain a seat for your student.
This form is a commitment and any changes have a negative impact on class size and program availability. 

Important information to understand:

  - This form must be submitted no later than January 27, 2017 in order to confirm your child’s placement.  

  - A separate Intent to Re-enroll form must be completed for each child in your family.
  - We require a completed online re-enrollment form and cannot accept phone confirmations.
  - If you need assistance completing this form, please come to the school and they will be happy to provide assistance with and/or access to the technology.

Your failure to complete this form will forfeit your student’s placement in this School of Choice or program.
Select the School to which you are re-enrolling:
If you have moved out of the West Ada School District boundaries, your current student(s) may continue to attend their current school of choice pending space available with priority given to families living within the West Ada Boundaries (see Policy 604.00).  In addition to this form, all out-of district students must complete the Out of District Transfer application and receive approval from a West Ada Regional Director (see Policy 501.90).  That form can be found on the School Registration and Enrollment page of the district website.
